- Please complete and save the AYF Participant Packet (PDF) before starting the online registration.
- All forms must be signed and saved as a PDF file.
Schedule a Sports Physical
Sports Physical must be completed on or after February 1, 2025
- Upload a scanned copy or clear photo of the completed physical page 6 that states FORM 15.7-B 03/27/2025 (rev.) in the bottom left corner for online registration.
- Physicals completed before Feb 1, 2025 will not be accepted.

Gather Required Documents
You’ll need the following to register:
Completed AYF Participant Packet- Sports Physical dated after Feb 1, 2025 (please have page 6 that states FORM 15.7-B at the bottom left)
- Birth Certificate (copy for verification)
Report Card (only required for players 13 and over, to be turned in at the fitting event — not uploaded online)

MANDATORY FITTING EVENT
All players must attend a fitting event.
- Dates and locations will be announced this spring and shared via email and our Facebook page.
- The fitting is important to determine uniform sizes, check out equipment and to insure that you get your uniform before the first game.
