Schedule a Sports Physical

- PREFERRED - Upload a scanned copy or clear photo of the AYF 2026 Medical Clearance Form
- OR Upload a scanned copy or clear photo of the completed AIA Sports Physical FORM 15.7-B
- Sports Physical must be completed on or after February 1, 2026

Completed AYF Participant Packet
Sports Physical dated after Feb 1, 2026
Birth Certificate (copy for verification)

Once you have all your documents ready:
- Click the button below to begin registration.
- You will upload your documents as part of the process.
All players must attend a fitting event.
- Dates and locations will be announced this spring and shared via email and our Facebook page.
- The fitting is important to determine uniform sizes, check out equipment and to insure that you get your uniform before the first game.
